Consent for Contact, Evaluation, Testing, Assessment, and Payment from The MMPI GUY.  Consent to Services I, the undersigned, hereby consent to receive evaluation, testing, and assessment services provided by The MMPI Guy, a board-certified clinical psychologist. I understand that these services include the administration and interpretation of the Minnesota Multiphasic Personality Inventory (MMPI) and any related consultations. Contact Consent I agree to allow The MMPI Guy to contact me via phone, email, or other provided contact methods for scheduling, providing results, and any follow-up consultations related to the MMPI evaluation and assessment. Payment Consent I understand that there are fees associated with the evaluation, testing, and assessment services. I agree to pay the full amount for these services as agreed upon prior to the administration of the MMPI. Payment methods is due at time of testing request. Confidentiality and Privacy I acknowledge that all information shared during the evaluation, testing, and assessment process will be kept confidential and used solely for the purpose of providing accurate and reliable psychological assessments.
